Authoritative Clinical Reference
Not Scheduled (OTC ā available without prescription)
Topical (Cutaneous)
Form Strengths
Gel 2.5%, 5%, 10%
Cream 2.5%, 5%
Face Wash / Cleanser 2.5%, 5%
FDC with Adapalene Adapalene 0.1% + Benzoyl Peroxide 2.5%
FDC with Clindamycin Clindamycin 1% + Benzoyl Peroxide 2.5%, 5%
INDICATIONS + DOSING ā FOR CLINICIAN USE ONLY
Primary Indications (Approved / Standard in India)
Parameter Details
Starting dose Apply 2.5% gel/cream as thin layer to affected area once daily in evening
Titration If tolerated after 2ā4 weeks, may increase to 5% formulation; further increase to 10% only if required and tolerated
Usual maintenance dose 2.5% or 5% gel/cream once daily
Maximum dose 10% formulation applied twice daily (rarely needed)
Clinical notes Apply to dry skin 20ā30 minutes after washing face. Lower concentrations (2.5%) are equally effective with fewer side effects than higher strengths. Wash hands after application. Warn patient about bleaching of fabrics and hair.
With Topical Retinoid (e.g., Adapalene):
Parameter Details
Starting dose Apply FDC (Adapalene 0.1% + Benzoyl Peroxide 2.5%) once daily in evening
Titration Not applicable for FDC; assess tolerance at 2ā4 weeks
Usual maintenance dose Once daily application
Maximum dose Once daily
Clinical notes Apply to dry skin. If using separate preparations, apply retinoid at night and benzoyl peroxide in morning to prevent oxidation and inactivation of retinoid.
With Topical Antibiotic (e.g., Clindamycin):
Parameter Details
Starting dose Apply FDC (Clindamycin 1% + Benzoyl Peroxide 2.5% or 5%) once daily
Titration Not applicable
Usual maintenance dose Once daily application
Maximum dose Once daily; avoid use beyond 12 weeks without clinical reassessment
Clinical notes Combination with benzoyl peroxide reduces antibiotic resistance risk. Reassess at 12 weeks ā if continuing topical antibiotic, benzoyl peroxide co-administration is essential.
Secondary Indications ā Adults (Off-label)
Indication Dose Duration Notes
Bacterial Folliculitis Apply 5% gel to affected areas once or twice daily 5ā7 days; may extend with caution OFF-LABEL. Based on Indian dermatology practice. Limited trial evidence.
Rosacea (papulopustular) ā adjunct Apply 2.5% gel once daily to affected areas Variable; specialist-guided OFF-LABEL. SPECIALIST ONLY. Evidence from small studies; use with caution due to irritation risk.
PAEDIATRIC DOSING (Specialist Only)
ā ļø Not recommended below 12 years of age except under specialist supervision (limited evidence; increased skin sensitivity in younger children).
Primary Indications ā Paediatric
Acne Vulgaris (Adolescents ā„12 years)
Parameter Details
Starting dose Apply 2.5% gel/cream as thin layer to affected area once daily in evening
Titration May increase to 5% after 2ā4 weeks if well tolerated; 10% rarely required
Usual maintenance dose 2.5% or 5% gel once daily
Maximum dose 5% gel twice daily (10% formulation generally avoided in adolescents)
Clinical notes Apply to dry skin only. Avoid broken, inflamed, or eczematous skin. Avoid contact with eyes, lips, and mucous membranes. Use sunscreen during daytime.
Safety Monitoring in Paediatrics:
Secondary Indications ā Paediatric (Off-label)
Indication Dose Duration Notes
Folliculitis (ā„12 years) Apply 2.5ā5% gel to affected areas once daily 5ā7 days OFF-LABEL. SPECIALIST ONLY. Based on dermatology practice.
No dose adjustment required.
Topical application with minimal systemic absorption; renal function does not affect drug disposition.
| Severity | Recommendation |
|---|---|
| Mild impairment | No adjustment required |
| Moderate impairment | No adjustment required |
| Severe impairment | No adjustment required |
Note: Not applicable ā negligible systemic absorption from topical cutaneous use.
Parameter Details
Risk category Generally considered safe for topical use (minimal systemic absorption)
Indian guidance May be used in pregnancy for acne management; preferred topical agent
Preferred alternatives Benzoyl peroxide is itself a preferred option; azelaic acid is alternative
When it may be used When topical acne treatment is clinically indicated; avoid large-area or extensive application
Monitoring Monitor for local irritation; no fetal monitoring required for standard use
Parameter Details
Compatibility Compatible with breastfeeding when used on limited areas
Drug levels in milk Negligible (minimal systemic absorption)
Preferred alternatives Benzoyl peroxide is itself a preferred topical acne agent during lactation
Application advice Avoid application on or near breast/nipple area
Infant monitoring Monitor for contact skin reactions if inadvertent infant exposure occurs
Parameter Recommendation
Starting dose 2.5% formulation once daily if used for adult-onset acne
Titration Slower titration; assess tolerance before increasing strength
Special considerations Elderly skin may be thinner and more prone to irritation; ensure no underlying dermatoses or skin atrophy
Clinical notes Acne is uncommon in elderly; if prescribed, use lowest effective strength
Interacting Drug/Agent Effect Recommendation
Tretinoin (topical) Benzoyl peroxide oxidises and inactivates tretinoin when applied simultaneously Do NOT apply together. Use at different times ā tretinoin at night, benzoyl peroxide in morning.
Other topical oxidising agents (hydrogen peroxide, salicylic acid in same application) Increased irritation; chemical instability Avoid simultaneous application
Interacting Drug/Agent Effect Recommendation
Topical retinoids (adapalene, tazarotene) Potential for increased irritation if applied together Use at different times of day; or use stable FDC (adapalene + benzoyl peroxide)
Topical clindamycin/erythromycin Additive effect; reduces antibiotic resistance May be used together; often co-formulated. Monitor for irritation.
Other topical acne agents (azelaic acid, salicylic acid) Cumulative irritation Stagger application times; monitor skin tolerance
Isotretinoin (oral) Additive dryness and irritation Use benzoyl peroxide cautiously; lower strength preferred
Adverse Effect Clinical Action
Severe contact dermatitis Discontinue immediately; treat with topical corticosteroid if needed
Allergic hypersensitivity reaction (angioedema, severe rash) Discontinue immediately; manage appropriately
Blistering or ulceration of skin Discontinue; allow skin healing before any topical therapy
Phase Parameters
Baseline Clinical assessment of skin type, acne severity, presence of dermatitis/eczema
After initiation Assess for irritation, erythema, peeling at 1ā2 weeks
Follow-up Clinical response at 4ā6 weeks; reassess need for dose titration
Long-term Reassess at 12 weeks if using with topical antibiotics; monitor for sensitisation or excessive dryness
